Please complete the online form below if you would like to request a Directory of Licensees. Once the form is completed and submitted, an invoice will be generated through our online payment processor, Authorize.net and emailed to you for payment. Once the invoice has been paid, the requested directory will be emailed to you in a MS Excel format. Disciplinary information is not included in the directory. The cost of the directory is $0.02 per licensee included in directory PLUS a nominal credit card processing fee of 2.8% + $0.25.)
