DVM APPLICATION REQUIREMENTS
APPLICATION STATUS - 1
Fourth Year Student, New Graduate, or DVM licensed and practicing for less
than 90 days
APPLICATION
REQUIREMENTS
1.
Complete, sign and notarize application form and submit to board office
2. Provide certification of anticipated graduation; OR
3. Provide proof of graduation from an accredited school of veterinary
medicine (copy of diploma or original final
transcript from school)
4. Have three (3) character reference forms submitted
EXAMINATION
REQUIREMENTS
5.
Take and pass NAVLE (or provide passing scores on previously taken NBE and CCT
or NAVLE)
6. Take and pass the Louisiana State Board Examination
PRECEPTORSHIP
REQUIREMENT
7.
Eight weeks (320 hours) in a Board approved private, clinical practice under
direct supervision of a licensed DVM

Licensed DVM practicing for less than five years
APPLICATION
REQUIREMENTS
1.
Complete, sign and notarize application form and submit to board office
2. Provide proof of graduation from an accredited school of veterinary
medicine (copy of diploma or original final transcript from school)
3. Provide proof of out-of-state licensure(s) in good standing
4. Have three (3) character reference forms submitted
5. Provide clean report from the National Disciplinary Database through AAVSB-VIVA
(is provided with national scores transfer)
EXAMINATION
REQUIREMENTS

Provide passing scores on national examination (NBE and CCT or NAVLE) taken no
more than five years prior to application
7. Take and pass the Louisiana State Board Examination
PRECEPTORSHIP
REQUIREMENT
8. Not required/waived if proof of employment in
# 7 above is provided; OR provide proof of employment as a licensed DVM
practicing in a private, clinical practice for not less than 90 days
immediately prior to application to waive eight-week preceptorship
requirement.
APPLICATION STATUS - 3
Licensed DVM practicing more than five years
APPLICATION
REQUIREMENTS
1.
Complete, sign and notarize application form and submit to board office
2. Provide proof of graduation from an accredited school of veterinary
medicine (copy of diploma or original final transcript from school)
3. Provide proof of out-of-state licensure(s) in good standing
4. Have three (3) character reference forms submitted
5. Provide clean report from the National Disciplinary Database through AAVSB-VIVA
(is provided with national scores transfer)
EXAMINATION

Take and pass the Louisiana State Board Examination
7. Provide passing scores on national examination (NBE and CCT) that are not
greater the five years old; if national examination score is greater than five
years old and in lieu of having to re-take and pass the national examination,
provide proof of employment as a licensed DVM practicing in private, clinical
practice or its equivalent for not less than the five years immediately
preceding application

REQUIREMENT

Not required/waived if proof of employment in # 7 above is provided; OR
provide proof of employment as a licensed DVM practicing in a private,
clinical practice for not less than 90 days
immediately prior to application

Foreign Veterinary School Student or Graduate
APPLICATION
REQUIREMENTS
1.
Complete, sign and notarize application form and submit to board office
2. Students must provide certification of anticipated graduation from
veterinary school
3. Provide proof of enrollment and completion of Step 3 in a Board accepted
foreign education certification program (AVMA-ECFVG or AAVSB-PAVE); OR
4. Provide proof of completion from a Board accepted foreign education
certification program (AVMA-ECFVG); a completion certificate must be submitted
prior to licensure issued
5. Copy of diploma from veterinary school
6. Have three (3) character reference forms submitted
