DVM Applicant Requirements

APPLICANT STATUS – 1 Fourth Year Student, New Graduate, or DVM licensed and practicing for less than 90 days

APPLICATION REQUIREMENTS
    Forms must be original unless otherwise stated.  Faxed forms not accepted.

1. Complete, sign and notarize application form and submit to board office
2. Provide certification of anticipated graduation; OR
3. Provide proof of graduation from an accredited school of veterinary medicine (copy of diploma or original final transcript from school)
4. Three (3) character references

EXAMINATION REQUIREMENTS

5. Take and pass NAVLE (or provide passing scores on previously taken NBE and CCT or NAVLE)
6. Take and pass the Louisiana State Board Examination

PRECEPTORSHIP REQUIREMENT

7. Eight weeks (320 hours) in a Board approved private, clinical practice under direct supervision of a licensed DVM

APPLICANT STATUS – 2 Licensed DVM practicing for less than five years

APPLICATION REQUIREMENTS
     Forms must be original unless otherwise stated.  Faxed forms not accepted.

1. Complete, sign and notarize application form and submit to board office
2. Provide proof of graduation from an accredited school of veterinary medicine (copy of diploma or original final transcript from school)
3. Provide proof of out-of-state licensure(s) in good standing
4. Three (3) character references
5. Provide clean report from the National Disciplinary Database through AAVSB-VIVA (is provided with national scores transfer)

EXAMINATION REQUIREMENTS

6. Provide passing scores on national examination (NBE and CCT or NAVLE) taken no more than five years prior to application
7. Take and pass the Louisiana State Board Examination

PRECEPTORSHIP REQUIREMENT

8. Provide proof of employment as a licensed DVM practicing in a private, clinical practice for not less than 90 days to waive eight-week preceptorship requirement

APPLICANT STATUS – 3 Licensed DVM practicing more than five years

APPLICATION REQUIREMENTS
     Forms must be original unless otherwise stated.  Faxed forms not accepted.

1. Complete, sign and notarize application form and submit to board office
2. Provide proof of graduation from an accredited school of veterinary medicine (copy of diploma or original final transcript from school)
3. Provide proof of out-of-state licensure(s) in good standing
4. Three (3) character references
5. Provide clean report from the National Disciplinary Database through AAVSB-VIVA (is provided with national scores transfer)

EXAMINATION REQUIREMENTS

6. Take and pass the Louisiana State Board Examination
7. Provide passing scores on national examination (NBE and CCT) that are not greater the five years old; if national examination score is greater than five years old and in lieu of having to re-take and pass the national examination, provide proof of employment as a licensed DVM practicing in private, clinical practice or its equivalent for not less than the five years immediately preceding application

PRECEPTORSHIP REQUIREMENT

8. Not required/waived if proof of employment in # 7 above is provided; OR provide proof of employment as a licensed DVM practicing in a private, clinical practice for not less than 90 days to waive eight-week preceptorship requirement

APPLICANT STATUS – 4 Foreign Veterinary School Student or Graduate

APPLICATION REQUIREMENTS
     Forms must be original unless otherwise stated.  Faxed forms not accepted.

1. Complete, sign and notarize application form and submit to board office
2. Students must provide certification of anticipated graduation from veterinary school
3. Provide proof of enrollment in a Board accepted foreign education certification program (AVMA-ECFVG or AAVSB-PAVE); OR
4. Provide proof of completion from a Board accepted foreign education certification program (AVMA-ECFVG or AAVSB-PAVE); a completion certificate must be submitted prior to licensure issued
5. Copy of diploma from veterinary school
6. Three (3) character references

EXAMINATION REQUIREMENTS

7. Take and pass NAVLE (or provide passing scores on previously taken NBE and CCT or NAVLE)
8. Take and pass the Louisiana State Board Examination

PRECEPTORSHIP REQUIREMENT

PRECEPTORSHIP REQUIREMENT IN LOUISIANA

All Applicants

An eight-week (320 hour) BOARD APPROVED preceptorship is a REQUIREMENT FOR LICENSURE IN LOUISIANA. This is not the same as an externship or internship done through your school of veterinary medicine. Your school’s program MAY OR MAY NOT qualify you for a waiver of the Board’s requirement.

This is a real licensing requirement. It is not just a paperwork “formality”. Plan to take this requirement seriously. If you request a review of your student experience, you must allow three to four weeks for processing. If the experience does not substantially meet the Board’s requirements, you may be asked to obtain additional hours up to the maximum of 320.

Our program has very specific guidelines and paperwork requirements. It may be quicker to use your intended employer as a preceptor. This is NOT legal in Louisiana unless the employer is approved by this Board as a preceptorship host practice AND you register with the Board office as a preceptee FIRST. Retroactive approvals are strongly discouraged. They take longer to process and may be denied which will delay your licensure.

Any person who performs surgery, makes a diagnosis, prognosis, or prescribes mediations or appliances in Louisiana is practicing veterinary medicine. To legally practice veterinary medicine in Louisiana , you must have a license to practice or be registered as a preceptee with the LOUISIANA BOARD. If you are not registered with us then you must have a Louisiana license to provide these services. Direct supervision is NOT A SUBSTITUTE FOR BOARD APPROVAL (see Rules 702 and 1031 of the Louisiana Veterinary Practice Act).
